Day Services

Case Managers assist each individual in developing personal plans that enable them to live full lives through choosing and realizing their own goals.  We educated the individuals we support about their rights and responsibilities, including the right to privacy and the right to make decisions about their lives and their care.  The goal is always for the people ewe serve to be active, independent members of their community.

Targeted Case Management

A Registered Nurse and a Licensed Practical Nurse are on staff to supervise and provide medications management, wellness monitoring and referrals to local health providers.
